IgG2 paraproteins in multiple myeloma patients: a report from a referral center.
IgG paraproteins are detectable in most multiple myeloma (MM) patients, and are associated with more aggressive disease. Our purpose was to assess their prevalence and clinical correlates in a consecutive series of MM patients evaluated for plasma cell dyscrasias at our institution. A retrospective review of 194 MM patients seen at a single institution from January 1994 to April 2006 was performed. Paraprotein type was determined in patients for whom purified protein derivative skin test was performed. IgG2 paraproteins were detected in 33 (17%) of 194 patients. A similar proportion of patients with IgG2 or IgG4 paraproteins had anemia (55% and 55%, respectively), compared with patients with IgG1 and IgG3 paraproteins (30%). IgG2 paraproteins were detected less commonly in patients with more than 1 myeloma associated monoclonal immunoglobulin (M-Ig). Patients with IgG2 paraproteins had less favorable outcomes than patients without IgG2 paraproteins (relative risk, 2.7). IgG2 paraproteins were detected in 17% of MM patients referred for paraprotein analysis, with similar proportions of patients having IgG2 paraproteins compared with patients in other studies. IgG2 paraproteins are associated with more aggressive disease.
